Organizations requesting funding must be a tax-exempt legal entity under IRS Section 501(c)(3) or another tax-exempt entity per IRS guidelines? This organization is not a private foundation as described under Section 509 (a).  Projects that take place outside of Centra’s service areas will not be considered.  Upload the applicable documentation.  If you have questions, please email Melinda Bunting at communityhealthsponsorships@centrahealth.com.

Frequently Used Method of Payment Forms

       1.   Electronic Funds Transfer (EFT) Form (Payment will be deposited into the bank account provided on the form)

       2.   AP Check Request Form (Payment will be sent to the address provided on the form and this will also serve as your invoice)

You can request a recommendation from someone in the community who is familiar with your organization and cause if you provide their email address.
